Mental Health Targeted Case Management services are to assist individuals (recipients) in gaining access to needed medical, social, educational and other services so that they can lead a mentally stable lifestyle.
What are Target Case Manager Services?
Targeted Case Management services, other known as a TCM, are individualized, person-centered, strength based, outcome focused and based on individual needs.

ELGIBILITY REQUIREMENTS
- Not receiving case management services from another provider or under a home and community-based service waiver.
- Not located in a hospital, nursing home, intermediate care facility for the developmentally disabled or institution for mental diseases.
- For individuals from 02-99+ years of age who require advocacy for and coordination of services to maintain or improve level of functioning .
- For children 2 years of age it is determined by level of functioning and needs on whether can qualify for services.
