Targeted Case Manager
EDUCATION:
Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development).
EXPERIENCE:
One (1) year minimum of full time or equivalent experience working with children and/or adults with serious emotional disturbances.
LICENSURE/CERTIFICATION:
Has completed or agrees to complete AHCA-approved mental health targeted case management training within three (3) months of initially providing Medicaid services.
COMMUNITY SERVED: Children and Adults
KNOWLEDGE AND TRAINING REQUIRED:
- Have knowledge of available resources in the service area for children with serious emotional disturbances
- Is knowledgeable of and comply with state and federal statues, rules, and policies that affect the target population
- Effective time management skills
- Ability to maintain confidentiality
- Good communication skills
- Understanding of cultural diversity
- Ability to travel
- Effective problem solving skills
- Accurate Assessment Skills
- Ability to use computer/tablet/PC, calculator, telephone system, fax/copy machine, and other needed machinery/electronics to accurately provide services to service population.
- Ability to perform telemedicine services when necessitated.
